Surgery closure over Christmas period

Wednesday 25th, Thursday 26th December 2019 and Wednesday 1st January 2020

For medication required over the Christmas period requests should be submitted by Friday 13th December and collected by Thursday 19th December

Extended Hours Access for Nurse Services

For Nurses From Tuesday 16th April 2019 Leith Hill Practice will offering nurse clinics outside the normal surgery times from 6.30-8.30pm.

Services on offer will include travel, asthma reviews, diabetic reviews, sexual health services, INR monitoring.

We hope this will meet the needs of our working patients who find it difficult to attend during normal opening hours 

 

 

 

New Patient Registrationregister_now

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. As a guide, the area covers Dorking town to the Horsham bypass and from Coldharbour, Ewhurst and Warnham to Norwood Hill.

We also require sight of photographic ID such as a passport or photo driving licence. For a new baby/child we will need to see the form FP58 issued by the registrar or a birth/adoption certificate.

New entrants to the UK are required to provide proof of settlement by way of a visa or other documentation.

When registering you will need to complete a registration form and (for patients aged 16 and over) a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All patients at this practice are allocated a named GP (this is usually your registered GP). You are still able to book appointments with any GP but your named GP is responsible for your overall care. Should you wish to know who this is please ask at Reception.

Medical treatment is available from the date of registration. Please contact Reception for further information.

The registration form and questionnaire must be completed fully and accurately and returned to the practice for processing prior to making an appointment. Failure to do so may result in delay to your registration.
